Our surgeons have privileges at AdventHealth Port Charlotte (2500 Harbor Blvd, Port Charlotte, FL 33952) and Murdock Surgery Center (1400 Education Way, Port Charlotte, FL 33948).
If you choose to go to an ER, please use AdventHealth Port Charlotte.

- Medical records request: complete the 2020 Medical Release (PDF) and return it to the office or fax it to (941) 391-6937. Requests normally take 7–10 business days.

Appointments
We know you have many choices when choosing a Surgeon in Port Charlotte, FL, so we have made requesting an appointment a simple process via our Web site or by calling our office at (941) 391-5102.
Insurance and Billing
Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own Surgeon.
Payment Options
We accept cash, checks, CareCredit, and major credit cards. Please see our Financial Coordinator for details. Online payments can be submitted anytime through our Payment Center.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice for office visits. If you need to reschedule a procedure or surgery, please give 48 hours notice. A fee may be charged if a patient fails to show up for an appointment without sufficient notice.
